What Is Home Health?

Home health care empowers individuals to stay in the comfort of their own homes and maintain a sense of independence while recovering from a surgical procedure, chronic condition, or minor illness.  

Do You Qualify?

To qualify for home health services, you must have a medical need for the skilled services of a nurse or therapist and meet Medicare’s definition of being homebound. This means that leaving your home requires taxing effort, causing you to leave your home less frequently.

You can be temporarily homebound due to an illness or injury. 

Amari Home Health Care, Inc. must receive orders from your doctor to provide home health services. Upon request from you or referral from your doctor, we will coordinate with your healthcare provider directly. 

We can provide home health services in assisted living facilities as well as private residences.  

  • Focuses on providing medical and non-medical services to individuals recovering from illness, injury, or surgery, or those with chronic health conditions.

  • Typically provided to individuals who are homebound and require skilled nursing care, therapy services, or assistance with activities of daily living.

  • Services may include wound care, medication management, physical therapy, occupational therapy, speech therapy, and personal care assistance.

  • Aimed at promoting independence, improving health outcomes, and preventing hospital readmissions.

  • Can be provided for a limited period as part of a recovery plan or on an ongoing basis for individuals with chronic conditions or disabilities.

Social Services: Our professional staff assists patients, their families, and/or caregivers with the traumatic impact of illnesses. They can assist in linking the patients with referrals to agencies that can offer or provide the resources needed for support or additional assistance. 

Skilled Nursing: Registered nurses or licensed practical nurses can administer medications, monitor vital signs, provide wound care, and coordinate care with other healthcare professionals including:

  • Personal care and hygiene needs

  • Enteral Feeding

  • Post Surgical Care
