We’re dedicated to providing specialized support for those facing dementia and honoring the service of our veterans. With tailored care and compassion, we aim to ensure comfort and dignity for these individuals throughout their journey.

Dementia Care Program

Caring for a loved one with dementia is often very difficult, with family members and caregivers experiencing high levels of both physical and emotional stress. To alleviate some of this anxiety, we have created a specialized care plan for patients diagnosed with dementia, which includes support and education for caregivers. 

Training provides tools to physicians, RNs, nursing assistants, social workers, chaplains, and volunteers who assist in increasing and maintaining a patient’s quality of life, improving pain and symptom management, and preparing Advance Care Planning.

Veteran Care Program

One out of every four individuals who are at the end of life in the United States is a veteran, and each veteran has unique needs at the end of life.

We have the dedication and commitment to providing veteran-centered education to staff and volunteers to ensure that veterans have access to quality end-of-life care. 

We strive to address the unique healthcare needs of veterans, both physical and emotional, including the psychological toll of war and how it impacts their end-of-life journey.

Care Team Members Include

  • Medical Directors

    Physicians with a specialty in pain and symptom management who collaborate with the hospice team to oversee the plan of care.

  • Registered Nurses

    Hospice care specialists and pain management experts are available 24/7 to help with any concerns.

  • Certified Home Health Aids

    Specially trained and available to assist patients with personal care and daily living activities such as bathing, dressing and grooming.

  • Medical Social Workers

    A medical social worker provides emotional support, counseling, and assistance with navigating healthcare resources and community services for patients and their families facing medical challenges.

  • Chaplains

    Spiritual Care Counselors who provide non-denominational faith support and bereavement support to patients, family, and staff members.

  • Volunteers

    Specially trained volunteers who provide friendly visits for companionship and relief care.
